How to Make an Appointment:
- Complete and return our New Patient Intake form, found HERE.
- Optional: Send copies of your child’s IEP, 504 plan, EI evaluation, or any other developmental assessments you’d like to share.
- Optional: Your child’s primary care provider may send a referral to us.
- We will contact you when your documents have been reviewed – this may take a few weeks!
- Feel free to check in with us by phone, text, or email if you have not heard from us within 30 days of submitting the Intake form.
- The child’s custodial parent or legal guardian should plan to attend the first appointment if at all possible. If someone else will be bringing the child, please contact our office to discuss.
Do You Need Transportation?
If your child has Medicaid, you may be able to get a free Medicab to your child’s appointment. Please call MedAnswering at 866-932-7740 for more information. Keep in mind you will need to call at least 3 days BEFORE your child’s appointment.
If you need help setting up Medicab transportation to your child’s developmental appointment, please contact our office as soon as possible.
Attention Parents:
Please arrive promptly for your scheduled appointment. Late arrivals cause schedule delays for your fellow patients at our office. If you are late to your appointment, we will likely need to reschedule your appointment. We require 24-hour notice for cancellations which must be received during business hours in order to avoid being charged a $200 no-show fee.
